What is Cardiac Rehabilitation
Community Medical Center’s Cardiac Rehabilitation Program is designed to enable you to regain your health, independence and overall well-being through a medically supervised regimen. The Cardiac Rehabilitation Program consists of three key components:
1. Exercise.
Medically supervised by a team of specially trained cardiology professionals.
2. Education.
We provide education on topics such as nutrition, stress management, medications, and heart disease.
3. Counseling and Support.
For you and your significant other to help succeed in attaining a more healthy, active lifestyle

What is my commitment?
Phase II Cardiac Rehabilitation can typically last up to 12 weeks. Graduates of Phase II may elect to continue their program through our self-monitored Phase III Maintenance Program.

Who is eligible for Cardiac Rehabilitation?
Patients who have been diagnosed with stable angina or who have had a heart attack, bypass surgery, heart valve repair or replacement, heart transplant, angioplasty or coronary stenting are eligible.

Will my insurance cover rehabilitation?
The insurance coverage for Phase II Cardiac Rehabilitation varies based on your specific diagnosis and carrier. It is important for you to check with your insurance carrier to determine your eligibility. Phase III Cardiac Rehabilitation is not covered by insurance. A small monthly fee is charged.
