Services for care related to advanced illness are covered for Medicare- and Medicaid-eligible patients, and by most Health Maintenance Organizations (HMOs), managed care companies, and private health insurers. Patients and family members establish relationships with the hospice care team. We share in the belief that caring and closeness are equally important throughout life, particularly in its final stages. Our interdisciplinary team focuses on the palliative nature of hospice care, which involves the active treatment of pain and the management of symptoms before they begin to dramatically affect daily living. Van Dyke Hospice and Palliative Care Center ensures that end-of-life decision making remains in the hands of patients and families – allowing them to make informed choices based on what is important to them. Hospice helps individuals live life to the fullest in familiar surroundings with their loved ones present.
