Hospice and Palliative Care Center has access to the broadest array of comfort-care services offered throughout New Jersey.

Services for care related to advanced illness are covered for Medicare- and Medicaid-eligible patients, and by most Health Maintenance Organizations (HMOs), managed care companies, and private health insurers.

A Comprehensive Team Approach

Patients and family members establish relationships with the hospice care team. We share in the belief that caring and closeness are equally important throughout life, particularly in its final stages.

Our interdisciplinary team focuses on the palliative nature of hospice care, which involves the active treatment of pain and the management of symptoms before they begin to dramatically affect daily living.

Making Important Choices

Van Dyke Hospice and Palliative Care Center ensures that end-of-life decision making remains in the hands of patients and families – allowing them to make informed choices based on what is important to them. Hospice helps individuals live life to the fullest in familiar surroundings with their loved ones present. It offers a complete approach to end-of-life care, including the emotional and spiritual needs of the patient and family.

Programs and services include:

  • Pain and symptom management to enhance quality of life and help ease the stress placed on family members.

  • Cardiac special care to provide active medical management to patients with advanced cardiac disease.

  • Advanced dementia care to provide comfort to patients and assist families with caregiving and decision making.

  • Jewish hospice service to offer physical, emotional, and spiritual care in a unique context that upholds Jewish culture, traditions, and values.

  • Bereavement support for the first 13 months after the loss of a loved one, providing individual phone calls, memorial services, and specialized bereavement support groups.

  • The Shining Stars Program to address the physical, psychological, and spiritual needs of newborns, children, and adolescents with advanced illness.

  • The Gardening Angels which offer Horticultural Therapy to provide a calm environment for both the patient and the caregiver. It gives them the opportunity to enjoy nature, gardening, floral arranging, and other related projects.

Inpatient Care

There are circumstances when an acute care setting is necessary for treatment and the effective management of pain and symptoms. The goal of the Van Dyke Hospice and Palliative Care Center Inpatient Unit at Community Medical Center is to provide patients with the highest possible level of support, and pain and symptom management, on a short-term basis.
