Helping you take an active role in controlling and managing your diabetes.

Comprehensive Team Approach
An expert team of certified diabetes educators—including a registered nurse and registered dietitian—conduct individual and group programs to empower people with diabetes to manage their care.  Patients benefit from the collaborative efforts of a team of specialists who focus on providing an educational experience that motivates an individual with diabetes to take an active and positive role in his or her diabetes treatment plan.  Individuals who care for and about the person with diabetes also play an integral part of the diabetes team. In addition, a summary of each visit is sent to your referring physician to ensure continuity of care.

Diabetes Self-Management Series
Four two-hour sessions, facilitated by a nurse and dietitian explore topics including:
• Patho/physiology of diabetes
• Monitoring/tracking blood sugar readings
• Treatment modalities
• Macro/micro nutrients
• Meal planning, portion control, dining out, heart healthy eating and cooking
• Individualized meal plans
• Prevention of complications and much more
• Free three month, one-hour follow up session

Convenient morning, afternoon and evening sessions available.

One-On-One Education
One-on-one consultation with a nurse or dietitian is available for:
• Individual meal plans
• Weight loss
• Other therapeutic diets
• Carbohydrate counting
• Insulin instruction*
• Meter teaching

*Insulin Pump Education Includes:
• Operation of pump
• Practice of saline runs
• Carbohydrate counting
• Development of carb-to-insulin ratio
• Pump initiation

Gestational Diabetes Education Program
One session, small group class addresses how gestational diabetes develops, link between diet and blood glucose, meal planning blood glucose monitoring.

Insulin Pump Support Groups
• Meets every other month
• Educational topics

Free Programs
• One-hour programs with specific topics

Medicare assignment accepted and classes are covered by most insurances. A prescription with a diagnosis code from your physician is required to make an appointment. The prescription must read: “Referred to MMC Center for Diabetes Education.”
