Meeting the Needs of Older Adults
For the past decade, Monmouth Medical Center's geriatric program
has been a recognized leader in the development of unique health
care services for seniors. In 1982, a multidisciplinary geriatric
team was established in response to the growing needs of older adults,
laying the foundation for the Greenwall Geriatric Program.
Today, the Greenwall Geriatric Program coordinates health and
social services for the elderly and their families in a community-teaching-hospital
environment, focusing on comprehensive inpatient and outpatient
care and education. It provides medical care for the older adult,
assessment of memory disorders, education and support for the caregiver
and the health professional.
Here's a look at the extensive network of services available through
the Greenwall Geriatric Program.
Offering comprehensive evaluations for adults 65 and older, the
Geriatric Health Center is directed by a board-certified geriatrician.
A team of health care professionals works with patients and their
primary care physician to provide a comprehensive assessment of
health care and social needs. Primary care is available for those
who need a physician.
Initial visits to the health center usually include a complete
physical examination, a review of all medication, laboratory testing
and the taking of X-rays. The health center is open from 8:30 a.m.
to 4:30 p.m. Monday through Friday, and there is on-call coverage
at nights and on weekends.

The Memory Disorders program of the Geriatric Health Center provides
a comprehensive evaluation of patients who exhibit signs or symptoms
of dementia. Serious memory impairment, also known as dementia,
occurs in as many as 20 percent of adults older than 75 and can
be due to such factors as Alzheimer's disease, stroke, alcohol abuse
or medication reactions or interactions.
Through careful analysis, the program's team of medical specialists
registered nurses certified in geriatric care and geriatric social
workers uncover the underlying condition of the disorder and recommend
appropriate treatment.
The program also offers ongoing social support services to help
patients and family members ease the burden of dementia. For more

The Community Outreach Program offers assistance to older adults
in Monmouth County. The program's care managers assess needs and
coordinate services for older adults, working closely with their
families and physicians to ensure that they receive the care that
best suits their individual needs.
The care managers are social workers who place special emphasis
on educating family members on caregiving issues, health or social
needs, evaluating the home for safety and assisting with long-term
planning.

As part of the Caregiver Support Program, educational programs are
held throughout the year on current issues for those who care for
older adults. The sessions, which explore different topics related
to aging, including legal and health issues, are offered free to
