Case Management (973) 322-5055 The Case Management Department works closely with patients, families, physicians and insurance carriers to ensure that the care delivered meets the patient’s needs. Your nurse manager assists in planning your treatment under the care of your physician, communicates with your insurance carrier to obtain coverage and educates you about the requirements and/or limitations of your insurance, not only for you hospital stay, but for your post-hospital needs, as well. The nurse manager will facilitate referrals for home care services, outpatient service, subacute care, rehabilitation, community resources, equipment and transportation.
